1. a) Any person applying to the Agency to take the measurement or mitigation Radon License Exam shall:

2. 1) Submit a complete and legible application form;

   1. 2) Provide proof of successful completion of the USEPA Radon Measurement Operators Course, or an equivalent indoor radon and radon progeny measurement course approved by the Agency.
3. b) No person shall take the Radon License Exam more than once in any 28 day period.

4. c) Persons who have a physical or mental disability that requires reasonable accommodation in accordance with the Americans With Disabilities Act (42 USC 101 et seq.) shall, in addition to subsection (a):

   1. 1) Submit a complete and legible application form 30 days prior to the exam date;
   2. 2) Provide a physician's statement of the disability that includes a list of accommodations that are needed to take the exam.
