1. a) Sealed sources or devices manufactured, labeled, packaged, and distributed in accordance with a license issued under 32 Ill. Adm. Code 330 or equivalent requirements of the U.S. Nuclear Regulatory Commission or an Agreement State.
2. b) Sealed sources or devices non-commercially transferred from an Agency, U.S. Nuclear Regulatory Commission or an Agreement State medical use licensee.
3. c) Teletherapy sources manufactured and distributed in accordance with a license issued under 32 Ill. Adm. Code 330 or equivalent requirements of the U.S. Nuclear Regulatory Commission or an Agreement State.
