1. a) be a licensed child care provider holding a valid day care license from DCFS (see 89 Ill. Adm. Code 406, 407 or 408).
2. b) regularly provide full-day, year-round child care services.
3. c) typically rely on private-pay tuition receipts and/or payments through DHS' Child Care Assistance Program (CCAP (see 89 Ill. Adm. Code 50)) for at least 25% of its revenues.
4. d) be open and receiving children at the time of application.
5. e) certify that its operating capacity has been reduced as a result of COVID-19 restrictions.
