801—10.5(35D) Applicant’s responsibilities.

1. 10.5(1)  Report any change in the applicant’s condition that could affect the previously determined level of care.
2. 10.5(2)  Report changes in mailing address, county or state of residency.
3. 10.5(3)  Provide additional information, verification or authorization for verification concerning the applicant’s circumstances, condition of health, and resources if required.
4. 10.5(4)  Participate in a preadmission evaluation for level of care if required.
