781—9.3(556) Forms.

1. 9.3(1)  Appropriate forms must be completed on paper or online (whichever is applicable for the requested actions) to perform the actions listed below. Current forms are available online at www.greatiowatreasurehunt.com or as supplied by the state treasurer’s office.

2. 9.3(2)  Forms required by the office include but are not limited to the following:

   1. a. Claim Form.
   2. b. Holder Reimbursement Form.
   3. c. Affidavit of Administration.

This rule is intended to implement Iowa Code chapter 556.
