561—17.5(458A) Organization reports.

1. 17.5(1)  Every person independently engaged in, or acting as a principal or agent for another entity that is engaged in, the production, storage, transportation (except railroad), refining, reclaiming, treating, marketing, or processing of oil or gas, or the exploration for or production of metallic minerals, shall file the following with the department:

   1. a. The name under which the business is being operated.
   2. b. The name and contact information of the person, business or businesses engaged in any of the above.
   3. c. The plan of organization.

   4. d. In case of a corporation:

      1. (1) The law under which it is chartered;
      2. (2) The names and contact information for any persons acting as trustees;
      3. (3) The name(s) of the manager, agent or executive; and
      4. (4) The names and contact information of officers.
   5. e. If business is conducted under an assumed name, the names and contact information of all owners in addition to the other information required.
2. 17.5(2)  Immediately after any change of facts stated in the report filed, a supplementary report shall be filed with the department with updated information.
