The authority will notify suppliers of information of the use that will be made of the information, which persons outside the authority might routinely be provided the information collected and identify which parts of the requested information are required and which are optional and will state the consequences of failing to provide the information. This notice may be given in the authority’s rules, on written forms used to collect the information, in contracts, in program guidance, verbally, or by other appropriate means.
