1. 1. Needed services that contribute to the maintenance of, or the increased independence of, older individuals who are blind;
2. 2. Capacity-building efforts, including collaboration with other agencies and organizations; and
3. 3. Outreach to promote community awareness, involvement, and assistance.

The application delineates expenditure of funds, establishes program goals, identifies the scope and extent of services, and defines a plan of operation. The application is submitted to the U.S. Department of Education, Rehabilitation Services Administration.
