Rule 609-10-.03. Change in Name

The licensee shall submit a certified copy of a marriage certificate, court order, or other certified documentation of legal name change, and the appropriate application and fee established by the Board in order to make a change of name. Cite as Ga. Comp. R. & Regs. R. 609-10-.03
