Rule 110-2-5-.06. Design Approval Agency Requirements

The commissioner shall consider an approved evaluation agency for approval as a design approval agency upon request, providing:

1. (1) The requesting agency has submitted to the commissioner not less than three different sets of acceptable manufacturer model plans.
2. (2) The request is accompanied by a letter (bearing notarized signatures) stating that the agency will accept full responsibility for codes compliance of any plans or changes thereto which it approves.
3. (3) A listing of personnel is provided showing compliance with the qualification requirements of ASTM Standard E-541-10 (7-9).
