Rule 110-2-11-.01. Application for Certification

A person, firm or corporation may apply to the commissioner for certification of an existing industrialized building. The request shall be accompanied by plans and specifications and shall state the:

1. (1) Proposed occupancy class
2. (2) Construction type
3. (3) Wind velocity in miles per hour
4. (4) Floor loads
5. (5) Roof live loads
6. (6) Height (in stories)
7. (7) Fire resistance of exterior walls
8. (8) Individual thermal resistance value (R-value) of walls, roof/ceiling and floors
9. (9) Date of original construction, where known
10. (10) The registration number of any existing insignia
11. (11) The construction code standards used in the original construction, where known
12. (12) The unit serial number, where known

13. (13) All items not in substantial compliance with the current construction codes

    Plans shall comply with Rule 110-2-8.
