Any person, entity, or health maintenance organization which violates a cease and desist order of the department under s. 641.3909 while such order is in effect, after notice and hearing as provided in s. 641.3907, shall be subject, at the discretion of the department, to any one or more of the following:

1. (1) A monetary penalty of not more than $200,000 as to all matters determined in such hearing.
2. (2) Suspension or revocation of the health maintenance organization's certificate of authority.
