Each application for registration shall be in writing and under oath to the department, in such form as the department may prescribe. The application shall include the following:

1. (1) The legal name and residence and business addresses of the applicant if the applicant is a natural person, or, if the applicant is a partnership, association, or corporation, the name of every partner, officer, or director thereof.
2. (2) The location of the principal office of the applicant.
3. (3) The complete address of any other locations at which the applicant proposes to engage in such activities since the provisions of registration apply to each and every operating location of a registrant.
4. (4) Such other information as the department may reasonably require with respect to the applicant or any money transmitter-affiliated party of the applicant; however, the department may not require more information than is specified in part II.
