The agency shall publish an annual report on or before January 1 of each year, which shall be available to the public and which shall include, but not be limited to:
- (1) A list by name and address of all nursing home facilities in this state.
- (2) Whether such nursing home facilities are proprietary or nonproprietary.
- (3) The rating of each nursing home facility.
- (4) The name of the owner or owners.
- (5) The total number of beds.
- (6) The number of private and semiprivate rooms.
- (7) The religious affiliation, if any, of such nursing home facility.
- (8) The languages spoken by the administrator and staff of such nursing home facility.
- (9) Whether or not such nursing home facility accepts recipients of Title XVIII (Medicare) or Title XIX (Medicaid) of the Social Security Act.
- (10) Recreational and other programs available.
