1. (1) In accordance with Section 559.952(8), F.S., after the first 180 days of licensure and every 180 days thereafter, so long as the license is active, each licensee shall submit a complete and accurate Bi-Annual Report to the Office by filing a Form OFR-559-FTS-004, Bi-Annual Report Form, electronically through the Office’s REAL System at www.flofr.gov. A completed Bi-Annual Report form must be received by the Office no later than forty-five (45) days after the conclusion of each 180-day period.
