All Group Credit Disability contracts shall conform to Section 627.558, F.S., and shall contain the substance of the following statutory provisions (as appropriate):

1. (1) 627.559 – Grace period.
2. (2) 627.560 – Incontestability.
3. (3) 627.561 – Application: statements deemed representations.
4. (4) 627.562 – Insurability.
5. (5) 627.563 – Misstatement of age.
