The Commission shall issue via an electronic permitting system a no-cost permit for the long-term anchoring of a vessel within the waters of this state upon receiving an electronic application, available at https://myfwc.com/boating/lt-anchoring-permit/ that includes the following information:

1. (1) For the vessel owner or operator:

   1. (a) Name.
   2. (b) Mailing address.
   3. (c) Telephone number.
   4. (d) E-mail address.
   5. (e) Birthdate.
   6. (f) Driver license number, if applicable.

2. (2) For the vessel:

   1. (a) Make.
   2. (b) Model.
   3. (c) Year.
   4. (d) Style.
   5. (e) Hull identification number.
   6. (f) Registration number or United States Coast Guard documentation, if applicable.
   7. (g) Vessel name, if applicable.

3. (3) Location where the vessel will be anchored:

   1. (a) Body of water.
   2. (b) GPS coordinates.
