- (1) Any person who currently holds a Florida Licensed Mental Health Counselor (LHMC) license and wishes to convert same to Counseling Compact license shall submit a completed application to the Board on Form DH5116-MQA, Application for Counseling Compact Home State Decalartion-Current Florida Licensees Only (Revised 12/2025), hereby adopted and incorporated by reference, which can be obtained from HYPERLINK "http://flrules.org/Gateway/reference.asp?No=Ref-19098"http://flrules.org/Gateway/reference.asp?No=Ref-19098, or the Board’s website at http://www.floridasmentalhealthprofessions.gov/resources.
- (2) Any person who currently holds a Licensed Professional Counselor (LPC) license issued by a Compact member state and wishes to declare Florida as their new home state under the Compact shall submit a completed application to the Board on Form DH5117-MQA, Application for Counseling Compact – Florida Privilege-Holder to Home State License Conversion (Revised 12/2025), hereby adopted and incorporated by reference, which can be obtained from HYPERLINK "http://flrules.org/Gateway/reference.asp?No=Ref-19099"http://flrules.org/Gateway/reference.asp?No=Ref-19099, or the Board’s website at http://www.floridamentalhealthprofessions.gov/resources.
