- (1) Any person desiring to renew an active status license to practice speech-language pathology or audiology, or a certificate as a speech-language assistant or audiology assistant, shall apply to the Department of Health. The application for renewal shall be made on a form, prepared and furnished by the Department.
- (2) The Department shall renew only those licensees or certificate holders who have completed the application form, remitted the appropriate renewal fee established by Rule 64B20-3.007, F.A.C., and who have demonstrated that they have met the appropriate continuing education requirements established by Rule 64B20-6.001, F.A.C.
