- (1) The application fee for inactive status shall be three hundred fifty dollars ($350.00) if the application and fee are received by the Department during the timeframe established by the Department as the timeframe for biennial renewal of licensure; otherwise a fee of three hundred and fifty dollars ($350.00) shall be imposed in addition to the application fee.
- (2) The fee for reactivation of an inactive license shall be three hundred fifty dollars ($350.00) plus the fee for biennial renewal of an active license as established in Rule 64B18-12.004, F.A.C., plus a fee of three hundred and fifty dollars ($350.00) if the application for reactivation is received by the Department at any time other than the timeframe established by the Department as the timeframe for biennial renewal of licensure.
- (3) The fee for biennial renewal of an inactive license shall be three hundred fifty dollars ($350.00) if the fee is received by the Department during the timeframe established by the Department as the time for such renewal; otherwise a delinquency fee of three hundred and fifty dollars ($350.00) shall be imposed in addition to the renewal fee.
