(1) Applicants for licensure shall submit form DH-MQA 5067 (7/2022), Application for Genetic Counselor, incorporated by reference and available at HYPERLINK "http://www.flrules.org/Gateway/reference.asp?No=Ref-15007" http://www.flrules.org/Gateway/reference.asp?No=Ref-15007.
(2) Applicants applying for a temporary license shall submit form DH-MQA 5066 (7/2022), Application for Genetic Counselor Temporary License, incorporated by reference and available at HYPERLINK "https://www.flrules.org/Gateway/reference.asp?No=Ref-15008" https://www.flrules.org/Gateway/reference.asp?No=Ref-15008. A temporary license shall be effective for two years from the date of issuance and may not be renewed.
