1. (1) Each health insurer shall include the following company and contact information when submitting a report required in this section to the Agency for Health Care Administration:

   1. (a) Name of company;
   2. (b) Florida company code assigned by the Florida Office of Insurance Regulation;
   3. (c) NAIC company code as assigned by the National Association of Insurance Commissioners;
   4. (d) Measurement year;
   5. (e) Covered lives in Florida on December 31 of the measurement year;
   6. (f) Year beginning continuous license in Florida;
   7. (g) Company website;
   8. (h) Company telephone number;
   9. (i) Contact name;
   10. (j) Contact title;
   11. (k) Contact address;
   12. (l) Contact direct telephone number;
   13. (m) Contact e-mail address;
   14. (n) Contact FAX number;
   15. (o) Survey vendor name;
   16. (p) Survey vendor organization;
   17. (q) Survey vendor telephone number;
   18. (r) Auditor name;
   19. (s) Auditor organization;
   20. (t) Auditor telephone number; and,

   21. (u) Designate plan type as:

       1. Health plans of health maintenance organizations as defined under Chapter 641, F.S.; or

       2. Health plans of health insurers defined under Chapter 627, F.S.

2. (2) The health insurer company contact information shall be reported in a text file as described in subsection 59B-14.004(8), F.A.C., using a tab between each data element.
