1. (a) Every managed care organization shall annually, on or before June 1, file with the Department a report covering the preceding fiscal year.

2. (b) Such report shall include:

   1. (1) A financial statement of the organization, including its balance sheet and receipts and disbursements for the preceding fiscal year; and
   2. (2) A statement explaining any material change in the information originally submitted pursuant to this title.
