6314.1 Each independent living program shall have at least one (1) local advisory committee.

6314.2 The membership of a local advisory committee shall:

1. (a) Consist of at least three (3) members;
2. (b) Include at least one (1) representative of the neighborhood in which the main facility or a residence is located, who is otherwise unaffiliated with the independent living program; and
3. (c) Include at least one resident who is eighteen (18) years of age or older.

6314.3 The local advisory committee shall:

1. (a) Meet with the administrator at the main facility at least quarterly to review programs, policies, citizen complaints, and police contacts;
2. (b) Inform CFSA in writing of any situation that a majority of the committee believes warrants correction and that the independent living program has failed to correct within a reasonable period of time after being notified by the committee; and
3. (c) Report annually to CFSA and the contracting entity concerning:
4. (1) The number of admissions;
5. (2) The number, outcome, and length of stay of planned and unplanned discharges;
6. (3) The staff turnover rate and efforts to reduce it; and
7. (4) Program effectiveness in meeting the needs of residents.

6314.4 The independent living program shall provide the local advisory committee with the information necessary to prepare the report required under § 6314.3(c).

6314.5 The independent living program may not provide the local advisory committee with identifying information regarding individual residents.

6314.6 The administrator and the local advisory committee shall hold a public meeting at least twice a year.

6314.7 The independent living program shall maintain copies of the minutes of the local advisory committee meetings.
