Each education program shall submit an annual report to the Director, due no earlier than January 1 and not later than March 1 of each year, in an approved format which shall:
- (a) Update program information;
- (b) Identify any major changes in the program during the year; and
- (c) Include the annual self evaluation described in § 531 of this chapter.
