3030.1 The following are the maximum per diem rates for medical services provided by the Department of Human Services to patients at Forest Haven and D.C. Village:

Forest Haven Intermediate	120.00
D.C. Village Intermediate	108.50
D.C. Village Skilled Nursing	133.50
