Each firm or corporation subject to sections 53-278c-1 to 53-278c-7, inclusive, of the Regulations of Connecticut State Agencies shall provide the Commissioner with the following information:

1. (1) Model name and serial number of each gambling device;
2. (2) Date, time and route of travel;
3. (3) Storage location and security measures taken to prevent public access;
4. (4) Persons with access to the gambling device and the uses to which such device shall be put; and

5. (5) Such other information as the Commissioner may require.

   Such information shall be submitted at the time of initial application or renewal and updated as necessary.
