(a) The following personal data on complainants, respondents, witnesses or other persons is or may be contained in discriminatory practice case files:
- (1) educational records;
- (2) financial records;
- (3) records of medical or emotional condition or history;
- (4) information concerning employment or housing history;
- (5) information concerning family or personal relationships;
- (6) information concerning reputation or character; and(7) other information which because of name, identifying number, mark or description can be readily associated with a particular person.
(b) The following other data is or may be contained in discriminatory practice case files on complainants, respondents, witnesses or other persons:
- (1) addresses;
- (2) telephone numbers;
- (3) social security numbers;
- (4) bank account numbers;
- (5) income tax information;
- (6) employment identification and payroll numbers; and(7) other information which because of name, identifying number, mark or description can be readily associated with a particular person.
