1. (a) A community-based organization whose program has been granted certification shall report to the department no later than November 1^st of each year in a manner prescribed by the department. The report shall include (1) the number of individual development accounts established and their status;

   1. (2) verification that deposits are being made by the account holders pursuant to the approved savings plan;
   2. (3) the balance and sources of funding in the community-based organization's local reserve fund, and (4) all matching fund activity.
2. (b) The community-based organization shall provide such additional information as required by the department.
