All applications shall include

1. (i) the name and address of the applicant;

   1. (ii) the name and address of the applicant's counsel, agent, or other representative, if any;
   2. (iii) the purpose for which the application is made;
   3. (iv) any statutes and rules which support the application;

2. (v) a complete and concise description of the activities, facilities, projects, or other actions for which the license, permit or registration is sought;

   1. (vi) any other information which the department may require; and

   2. (vii) any additional information which the applicant considers relevant.

      Applications shall be addressed to the appropriate division of the department and shall be sent by mail or hand-delivered during normal business hours.
