(a) Routine RequestsAny person may request and receive the following information about a family child care home from the Office on a routine basis:
- (1) License status, which indicates whether the family child care home is unlicensed, licensed, applying for a license, or is no longer licensed due to suspension, revocation, voluntary withdrawal or surrender;
- (2) License number;
- (3) License capacity of the family child care home;
- (4) Expiration date of the license;
- (5) Listing of substantiated complaints against a provider during the past three years, excluding complaints for child abuse and neglect;
- (6) The date of the last inspection made by the Office; and(7) The status of any existing corrective action plan required to bring the provider into compliance with regulations.
