The bill form used shall show:

1. (1) The name of the utility furnishing the service;
2. (2) the reading of the meter at the end of the period for which the bill is rendered;
3. (3) the present and previous meter reading dates;
4. (4) the number and kind of units metered;
5. (5) the applicable rate schedule, or identification of the applicable rate schedule;
6. (6) the gross or net amount of the bill;
7. (7) the date by which the customer must pay the bill in order to benefit from any discount and to avoid any penalty;
8. (8) a distinct marking to identify an estimated bill;
9. (9) any conversions from meter reading units to billing units or, in lieu of such information, a statement advising that such information can be obtained by contacting the utility's prinicipal office;
10. (10) the address or post office box where payment may be made;
11. (11) telephone number of officers where information may be obtained.
