(a) The following individuals may make application for either a removable windshield placard or special license plates:
- (1) Any person who is blind or such person's nominee; and(2) Any person with disabilities or such person's nominee;
- (b) An application shall contain such information as required by the commissioner.
- (c) Each application by or for an individual person shall include certification of blindness or disability as provided in section 14-253a-18 of the Regulations of Connecticut State Agencies. The certification shall remain valid through the expiration date of the applicant's or nominee's credential.
