1. (a) Each person who applies for an identification card that the commissioner is authorized to issue in accordance with section 1-1h of the Connecticut General Statutes shall certify to the commissioner that he or she does not currently hold a valid license issued by any jurisdiction or foreign nation. The commissioner shall not issue an identification card to any person who holds a valid license.

2. (b) If the commissioner issues an identification card the department shall maintain a record for the holder which shall include:

   1. (1) full legal name and name history;
   2. (2) birth date;
   3. (3) gender;
   4. (4) address;
   5. (5) the date of card issuance and expiration;
   6. (6) card number; and(7) the date of cancellation, if any.
