The record before the presiding officer shall include:

1. (1) All motions, applications, petitions, pleadings, notices of hearing and intermediate rulings;
2. (2) The evidence received by the presiding officer;
3. (3) Questions and offers of proof, objections and rulings thereon during the hearing;
4. (4) The decision, opinion or report by the presiding officer.
