An application for a grant under chapter 173 of the Connecticut General Statutes shall include:
- (1) One copy of the educational specifications for the school building project;
- (2) One copy of the approval of the educational specifications by the local or regional board of education or governing body of the applicant;
- (3) One certified copy of a resolution adopted by the legislative body of the applicant establishing a school building committee for the school building project;
- (4) One certified copy of a resolution adopted by the legislative body of the applicant authorizing the preparation of at least schematic drawings and outline specifications for the school building project;
- (5) One certified copy of a resolution adopted by the legislative body of the applicant authorizing application for and acceptance of such grant;
- (6) Evidence of local funding authorization;
- (7) A full enrollment report with projection of at least eight years;
- (8) A professional cost estimate that includes evidence, sufficient to the Commissioner, that such estimate is based upon cost containment principles and value engineering;
- (9) A copy of the Grant Application Phase Cost Estimate completed through the Department’s BizNet website or a successor system identified by the Department;
- (10) A notification that the school building project includes the purchase of a site or existing building;
- (11) For applications to correct safety, health and other code violations under subsection (b) of section 10-283 of the Connecticut General Statutes, a copy of the citation or other official determination that the underlying condition is in violation of applicable state or federal requirements; and
- (12) Any other documentation required by the Department to determine eligibility.
