Colo. Rev. Stat. § 39-11.5-102
(1) At any time at least three years from the date of the sale of a tax lien pursuant to article 11 of this title 39, a lawful holder may file an application for a public auction of a certificate of option for treasurer's deed for the property subject to the tax lien described in the certificate of purchase held by the lawful holder. In so doing, the lawful holder shall file an application for public auction in a form and manner determined by the treasurer as follows:

The undersigned, as the holder of Treasurer's Tax Lien Sale Certificate of Purchase No.________ issued pursuant to the tax lien sale held on the ____ day of ________, 20__, for the taxes and/or special assessments for the tax year ______ hereby request that you, as County Treasurer, give notice and take such proceedings as are required by section 39-11.5-102, C.R.S., so that the undersigned may begin the process to obtain a Treasurer's Deed to the property described in said Treasurer's Tax Lien Sale Certificate, more particularly described as follows, to-wit:
