- (1) In addition to any other method permitted by the commission or office, a person may submit a request for evaluation by mail or online. The office shall develop an online request for evaluation form that is accessible from the commission's public website.
- (2) The commission and office shall permit a person to submit a confidential or anonymous request for evaluation.
