All providers with a pending application or currently on the List must notify the Director in writing within ten business days if:

1. (a) The provider's contact information has changed;
2. (b) Any specific limitations in providing pro bono legal services under § 1003.63(b)(4), (c)(2), or (d)(2) have changed; or
3. (c) The provider is no longer eligible under § 1003.62.
