Each inspection, grade, class, weight or combination certificate issued under the Act by a licensee to perform such services must be:

1. (a) In a format prescribed by AMS;
2. (b) Issued and maintained in a consecutive order; and
3. (c) As prescribed in the applicable licensing or provider agreement and authorized by AMS.
