Upon request, there shall be issued a cotton classification memorandum which shall embody within its written or printed terms:

1. (a) The results of the classification or comparison.
2. (b) The name of the country in which the cotton was produced.
3. (c) The source from which the samples were received for classification.
4. (d) A statement that any classification made has been on the basis of the Universal Cotton Standards (the official cotton standards of the United States) at the time of such classification.
5. (e) A statement that any classification made applies only to the samples as submitted by the owner or the owner's agent and does not purport to represent any particular cotton bales.
6. (f) The signature of the Director of the facility providing the classification service and the date of issuance of the memorandum.
