The notice required by § 840.3 shall include the following information:

1. (a) Name and title of person reporting.
2. (b) Name of railroad.
3. (c) Location of accident (relate to nearest city).
4. (d) Time and date of accident.
5. (e) Description of accident.

6. (f) Casualties:

   1. (1) Fatalities.
   2. (2) Injuries.
7. (g) Property damage (estimate).
8. (h) Name and telephone number of person from whom additional information may be obtained.
