45 C.F.R. § 1149.44 – What must documents filed with the ALJ include?

(a) Documents filed with the ALJ must include:
1. (1) An original; and
2. (2) Two copies.
(b) Every document filed in the proceeding must contain:
1. (1) A title, for example, “motion to quash subpoena”;
2. (2) A caption setting forth the title of the action; and
3. (3) The case number assigned by the ALJ.
(c) Every document must be signed by the filer, or his/her representative, and contain the address or telephone number of that person.
