The UR plan must provide that written notice of any adverse final decision on the need for continued stay under § 456.436 (g) through (i) is sent to—

1. (a) The ICF administrator;
2. (b) The attending physician;
3. (c) The qualified Intellectual Disability professional, if applicable;
4. (d) The Medicaid agency;
5. (e) The beneficiary; and
6. (f) If possible, the next of kin or sponsor.
