Permit reports must contain the following information as appropriate:

1. (a) Permittee(s)' name, title, affiliation, and professional contact information;
2. (b) Permit number;
3. (c) Date of report;
4. (d) Project name, number, or reference;
5. (e) Description of project, methodology, or summary of research scope of work;
6. (f) Dates of field work;
7. (g) Name(s) of people who performed field work;
8. (h) Description of work performed or accomplished and a summary of results and discoveries;
9. (i) Summary of regional or local geology and/or paleontology including context, geography, stratigraphy, and geological unit;
10. (j) Identification of potential impacts to paleontological resources by proposed land use action;
11. (k) Mitigation recommendations to address potential paleontological resource impacts;
12. (l) Relevant literature citations;
13. (m) Relevant associated records, including anything that aids in explaining, clarifying, or understanding the findings;
14. (n) Listing of collected paleontological resources, including field numbers and field identifications that are referenced to specific localities;
15. (o) Repository name, identifying acronym, and address;
16. (p) Repository official name, title, and contact information;
17. (q) Approved repository accession and/or catalog number(s);
18. (r) Assigned locality numbers;
19. (s) Administrative area (State, county, ranger district, forest, and so forth);
20. (t) Map name, source, size, edition, projection, datum, and/or other mapping information;
21. (u) Geographic location, survey data, and/or related metadata;
22. (v) Paleontological taxa collected, observed, or in a repository;
23. (w) Resource identifications, condition, location, and quantity; and
24. (x) Recommendations or information for the approved repository regarding the condition or care of collected resources or associated records.
