Each State agency designated in § 363.11(a) must collect and report separately the information required under 34 CFR 361.40 for—

1. (a) Eligible individuals receiving supported employment services under this part;
2. (b) Eligible individuals receiving supported employment services under 34 CFR part 361;
3. (c) Eligible youth receiving supported employment services and extended services under this part; and

4. (d) Eligible youth receiving supported employment services under 34 CFR part 361 and extended services.
