The payor Federal Reserve Bank will issue a replacement fiscal agency check if:

1. (a) You submit written notice:
2. (b) The check is unpaid;
3. (c) We determine that recovery of the original check is unlikely; and
4. (d) The payee and endorsee, if any, of the check execute the required indemnification agreement.
