(a) Each manufacturer subject to this Order may be required to provide to the Board periodically such information as may be required by the Board, with the approval of the Secretary, which may include but not be limited to the following:

1. (1) Number and type of concrete masonry units manufactured;
2. (2) Number and type of concrete masonry units on which an assessment was paid;
3. (3) Name and address of the manufacturer; and
4. (4) Date assessment was paid on each concrete masonry unit sold.
