The individual program plan planning team, shall, pursuant to Section 4646, meet, when it is necessary to review any of the following:

1. (a) The appropriateness of job placement.
2. (b) The appropriateness of the services available at the Work Activity Program or Supported Employment Program.
3. (c) The individual habilitation service plan.
