A claim must set forth, under oath, on the form prescribed by the commissioner:

1. (a) The particulars thereof, and the consideration therefor.
2. (b) Whether said claim is secured or unsecured, and, if secured, the nature and amount of such security.
3. (c) The payments, if any, made thereon.
4. (d) That the sum claimed is justly owing from such person to the claimant.
5. (e) That there is no offset to the claim.
6. (f) Such other data or supporting documents as the commissioner requires.
